The word approved looks precise until a claim passes from one person to another.

An email may say that a fee is approved. A note may say that a document has been cleared. A tracker may mark a request as complete. The next person still has to ask what the decision covered, whether it had a limit, which conditions remain, and what should happen next.

This is a different problem from collecting a missing document. The information exists. The weakness is in how the decision was recorded.

I have been thinking about this as a claims coordination problem. Insuveo may help collect information around a claim, but collection is only useful if the system can preserve the meaning of the decisions made along the way.

Approval always has an object

The word can refer to several decisions inside the same claim.

A team may approve the appointment of an external professional. It may consent to a cost up to a stated amount. It may accept a document for review. It may agree that a proposed action can proceed. It may authorize a payment after another condition is met.

Those decisions can be related, but they are not interchangeable.

If a system stores only approved: yes, the next person has to reconstruct the object of the approval from messages and attachments. A handoff then carries the status forward while losing the meaning behind it.

The basic record should answer a complete sentence: who approved what, for which purpose, within what scope, subject to which conditions, and based on which source?

A binary status can hide the remaining work

Consider a request for consent to incur a professional expense during a claim. The response may permit a specific piece of work, set a fee limit, require an invoice, or reserve a later review. Recording that response as approved makes the request look finished even when several actions remain.

The person managing the claim may still need to confirm the engagement, obtain the document, review the invoice, or submit the expense for reimbursement. Another participant may read the same status and assume payment has already been authorized.

The problem is not that anyone ignored the decision. The tracker compressed a conditional decision into a single word.

This compression can also happen when a reply contains mixed outcomes. One part of a request may be accepted while another needs more evidence. A cost may be allowed within a limit while the amount above that limit remains open. A document may be received without resolving the issue it was meant to support.

The system should keep those differences visible.

The approval record should preserve the decision

I would model an approval as a separate record with more detail than a label attached to the claim.

At minimum, that record would contain:

  1. The exact request that needed a decision.
  2. The person or role that made the decision.
  3. The scope covered by the decision.
  4. Any amount, period, or other limit.
  5. The conditions that still have to be met.
  6. The message or document that supports the status.
  7. The time of the decision and any later revision.
  8. The next action and its owner.

This structure gives a colleague enough information to continue the work without rereading the full thread. It also makes partial approval possible. A request can remain open where the source does not support a complete answer.

The source link matters because summaries can become more certain than the underlying message. A short note may say approved even when the original response used narrower language. Keeping the source beside the structured record lets the operator verify the interpretation.

Claims work needs several visible states

A useful workflow probably needs more than open and closed.

A request could be drafted, submitted, under review, approved with conditions, partly approved, declined, or reopened after new information. The exact states will vary by team. The important part is that each state describes what happened and what can happen next.

The workflow should also separate a decision from its execution. Consent to proceed, completion of the approved work, invoice review, and payment are different events. Combining them produces the same ambiguity that the approval record was meant to remove.

I do not yet know how much of this structure claims teams would want to maintain manually. Too many fields can become another administrative burden. The product would need to create most of the record from the existing conversation while asking a person to confirm the decision.

AI should propose the record and leave authority with the team

An AI system could identify a likely approval in an email or document, connect it to the original request, extract conditions, and suggest the next action. That would reduce the work of updating a tracker after every reply.

It should not infer approval from a friendly or positive response. It should not expand a narrow consent into a broader one. When the source is ambiguous, the record should say that human confirmation is required.

The review screen could show the original request, the relevant source text, the proposed scope, any detected limits, and the action that would follow. A claims professional would confirm or correct the record before the workflow continued.

That is a narrower role for automation than deciding coverage, liability, or settlement. It still has value if it prevents a later participant from acting on a status that lost its conditions.

The first test is whether another person can take over

This idea is still a product hypothesis. A conversation about confusing approvals does not establish demand, frequency, or savings.

I would test it on one recurring claims coordination process. For each decision, the team would compare the current record with a structured approval record. Then a colleague who was not part of the original exchange would try to answer four questions: what was requested, what was decided, what remains conditional, and what happens next?

If the structured record does not make those answers faster and more reliable, the extra model is not useful. If it does, the next question is whether the benefit justifies changing the team's existing workflow.

The first test should therefore measure whether the next person can continue without reconstructing the approval from messages. The record is useful only if it keeps the scope and source attached to the decision.
